What is the story behind it?
From the eighth canto of the Bhagavata: the elephant Gajendra, seized by a crocodile in a lake and exhausted after a long struggle, finally gives up fighting and prays. Vishnu comes at once. The hymn is the prayer he made.
What is it recited for?
Release - from litigation, debt, addiction, illness, any grip that will not loosen. The tradition uses it precisely where a person has struggled and failed, because that is the situation the hymn was spoken in.
What does the prayer actually say?
It does not ask to be saved from the crocodile. Gajendra praises the Lord who is the cause of all and to whom nothing is owed - and the surrender itself is what ends the struggle. That reversal is the teaching of the passage.
When is it recited?
On Ekadashi and Thursday, and daily by someone in a prolonged difficulty, often as a sankalpa. Many read it at a sickbed, since Gajendra's exhaustion reads so directly.
Which other hymn is like it?
The Narasimha Karavalamba Stotram, which asks in the same register for a hand to be extended. The two are often recited together in hard times.
