What is this stotram?
The Sankashta Nashana Ganesha Stotram from the Narada Purana - eight verses naming Ganesha in twelve forms, from Vakratunda to Dhumravarna, and asking that trouble be destroyed. Sankat nashan means exactly that.

When is it recited?
Wednesdays and the monthly Chaturthi, through Ganesh Chaturthi, and at the start of any difficult undertaking. It is also read during Sankashti Chaturthi, whose vrat is set out on the Sankashti vrat vidhi page.
What are the twelve names for?
They are the twelve traditional forms of Ganesha, and the hymn's phalashruti says that reciting the names removes fear, debt and obstruction. Naming a deity in his forms is the standard mode of a Puranic stotram.

A stotram is a longer Sanskrit hymn of praise, not a forty-verse chalisa and not the lamp-song of an aarti. Browse every stotra on the Stotram index. For japa of a short formula use a mantra. The steps of worship around the recitation are in Puja Vidhi.
