How is this different from Gandanta, Maraka or Pushkara Bhaga?
Gandanta is the junction of fire and water signs and of certain nakshatras. Maraka Dosha is the 2nd and 7th houses as death-inflicting houses. Pushkara Bhaga is a nourishing degree; this site occupies that whole numbered degree (for example 21° 00' to 21° 59'). Mrityu Bhaga is a different table and uses the Nth-degree span (N-1)° to N°.
Why does birth time matter, and how is Gulika placed?
Each band is only one degree, so a few minutes can move Lagna or a fast graha across it. Gulika (Mandi) uses the same Saturn kala as the Gulika calculator - the sidereal Lagna at the start of Saturn's eight-fold portion. At high latitudes where sunrise is unreliable, Gulika is omitted and the rest of the chart is still scored.
Do Rahu, Ketu and Lagna count?
Yes. Jataka Parijata lists degrees for Lagna, the seven grahas, Rahu, Ketu and Mandi (Gulika). All of those are checked. The natal chart itself is in Kundli Maker.

Mrityu Bhaga is the death-inflicting or sensitive degree of a graha, the Lagna or Gulika (Mandi) in a given rasi. Jataka Parijata (Adhyaya 1, Shloka 57) and Sarvartha Chintamani list one integer per body per sign. That integer N is the Nth degree of the sign: from (N-1)° 00' up to N° 00'. Sun in the 20th degree of Aries occupies 19° 00' to 20° 00', not 20° 00' to 20° 59'. That counting differs from Pushkara Bhaga on this site, which occupies the whole numbered degree.
